What is it

What is All-on-X (All-on-4 / All-on-6)?

All-on-4 and All-on-6 are ways of supporting a fixed prosthesis on a small number of implants (4 or 6) when all the teeth in a jaw are missing. Instead of a removable denture, the aim is a solution that stays fixed in the mouth. If you are missing one or a few teeth, see our Dental Implants page.

In a systematic review of the All-on-4 concept (Patzelt et al., Clinical Implant Dentistry and Related Research, 2014), implant survival was very high in the first years (up to three years of follow-up); longer-term data are more limited. Results vary from person to person; suitability is confirmed only by an examination and 3D CBCT imaging.

Good planning starts with avoiding unnecessary procedures.

Whether All-on-4 or All-on-6 is right for you is not promised in advance. The condition of your jaw bone and your chewing needs are assessed on 3D CBCT imaging, and the plan is built for your individual case.

Treatment options when all teeth are missing

When all the teeth are missing there are two main approaches; the real difference is whether the prosthesis is fixed to the jaw.

In full-mouth cases the conventional method is the complete denture (commonly known as false teeth) — a removable prosthesis that rests on the jaw. The current approach is the implant-supported fixed prosthesis (All-on-4 / All-on-6): the prosthesis is fixed onto implants placed in the jaw and stays in the mouth permanently. That said, where jaw bone is insufficient, where surgery may carry a general-health risk, or for economic reasons, the complete denture also remains an appropriate option at our clinic. Which method fits your case is assessed together after an examination and 3D CBCT imaging.

An implant-supported fixed prosthesis (All-on-4 / All-on-6) stands out for comfort and chewing in many cases; but where bone is insufficient, general health carries risk, or economics matter, a complete denture is also a sound solution. Which one you receive is not promised in advance; the decision is made individually after 3D CBCT imaging and an examination. Our aim is to plan what you genuinely need — not what you want to hear.

The process

How does the treatment work?

The steps below show the general flow; timing and stages can vary from person to person.

Initial assessment

Review of your overall situation and your expectations.

3D CBCT scan

Bone analysis, nerve canals and the planning of implant angles.

Surgical stage

The implants are placed under local anaesthesia.

Provisional fixed prosthesis

In suitable cases, a provisional fixed prosthesis is fitted at an early stage.

Final fixed prosthesis

After healing (osseointegration), the final fixed prosthesis is fitted.

Digital protocol

Fixed provisional teeth — often within days, in suitable cases

Because 3D CBCT imaging is available in-house, in suitable cases the path from planning to treatment is kept short and predictable. The flow below is a general example; timing varies from person to person.

Day 1

The full operation is carried out and the implants are placed in the jaw.

Day 2

Swelling and discomfort are typically limited; your digital scan (impression) is taken.

Day 3

Your provisional fixed teeth are fitted, and we say goodbye until your three-month review.

At 3 months

The digital scan is repeated and you make a fresh start with your final teeth.

Back to daily life — often within a few days.

In suitable cases, many patients return to daily life within a few days, initially with fixed provisional teeth. After the permanent prosthesis — usually around three months — most patients regain comfortable eating and chewing. Timing and outcomes vary from person to person.

Healing & aftercare

Healing, aftercare and how long it lasts

With planned treatment, post-operative swelling and sensitivity are usually mild and are managed with the aftercare your dentist recommends. Long service depends on regular oral hygiene and clinical check-ups.

Why is the exact system chosen case by case? You agree the tier with us before you travel. Within that tier, each implant system has its own surface, connection and geometry; the right system and line can only be selected after an in-person examination and a 3D CT scan (CBCT), taking bone volume and the needs of the case into account. Each implant is covered by its manufacturer's guarantee.

The crown on every tier is made from monolithic multilayer zirconia: a CE-marked block from a manufacturer based in the EU, Japan or the USA, and the same material whichever tier you choose. The exact brand is named in your written treatment plan, and the block lot number is recorded in your patient file.

FAQ

Common questions

In suitable cases, a provisional fixed prosthesis can be fitted shortly after the implants are placed (for example, within a few days). The final prosthesis is fitted after the implants have fused with the bone (osseointegration). Whether this is possible for you is determined after a CBCT scan and an examination.

Neither is absolutely superior to the other; the choice depends on the condition of the jaw bone and your chewing needs. All-on-4 can be sufficient where the bone structure is suitable, while All-on-6 is preferred where more support is needed. The decision is individual, made after a CBCT scan.

The procedure is carried out under local anaesthesia; pain is generally not felt during treatment. Mild swelling or sensitivity may follow for a few days; this is managed with the aftercare your dentist recommends.

We answer this honestly. Before you travel home you receive an implant passport — a written record of the implant brand, model and materials used — so any dentist, anywhere, knows exactly what is in your jaw, and the implants themselves carry the manufacturer's guarantee. After you fly home we stay in contact through a remote follow-up: you can send photos and we review them and advise. We do not promise a named partner dentist in your country, because we will not claim a network we do not have.

We work with three tiers. Essential uses Neodent (Brazil, Straumann Group) or Osstem (South Korea); Select uses Medentika (Germany, Straumann Group); Reserve uses Straumann (Switzerland). The tier is the only difference between our price levels — the zirconia crown is the same on each — and you agree it with us before you travel. Within a tier, the exact system and line are decided after the examination and 3D CT scan, and it is written into your treatment plan so any dentist can identify it later. Each implant is covered by its manufacturer's guarantee.

We only use CE-marked zirconia blocks from manufacturers based in the EU, Japan or the USA, and the same material is used on every price tier — a lower-priced implant tier does not mean a different crown. We deliberately do not tie our website to a single brand name: the block is supplied by our laboratory partners, and we would rather tell you exactly what was used than promise something a third party could change. So the brand is written in your treatment plan before you travel, and the block's lot number is kept in your file so any dentist can identify it in the years ahead.
